The initiative promotes multidisciplinary collaboration, shared data use, and innovation to improve care quality, health equity, and workforce development. Through two focused work packages, the project builds a sustainable research ecosystem and pilots real-world solutions to enhance primary care effectiveness. The expected outcomes include better treatment results, reduced healthcare costs, and a more skilled and resilient healthcare workforce.
