Description of the granted funding

The PHC Health Hub project is transforming primary health care in Eastern Finland by establishing a cutting-edge Research, Development, Innovation, and Education platform at the Teaching Clinic Osmo. In collaboration with regional wellbeing services counties, universities, and stakeholders, the project integrates research and education directly into clinical practice. At its core, Osmo functions as a novel academic health center where professionals teach and conduct research on-site, ensuring rapid application of new knowledge. The initiative promotes multidisciplinary collaboration, shared data use, and innovation to improve care quality, health equity, and workforce development. Through two focused work packages, the project builds a sustainable research ecosystem and pilots real-world solutions to enhance primary care effectiveness. The expected outcomes include better treatment results, reduced healthcare costs, and a more skilled and resilient healthcare workforce.
